Loneliness Prevention Project Co-ordinator
AFC Fylde Community Foundation is looking to recruit a project co-ordinator for their Fylde Loneliness Prevention Project that will support older people in the Fylde borough to reduce social isolation and loneliness.
The successful candidate will co-ordinate and deliver the project working alongside local partners to empower older people to take control of their health and wellbeing.
The role is funded by The National Lottery Community Fund and will see the co-ordinator provide individuals with a holistic one to one support programme and connect them with community groups and statutory services. They will also support existing groups to be accessible and sustainable and help people to start new community groups, working collaboratively with all local partners.
Length of Contract: Fixed term until 31st March 2026
Salary: £24,000 - £26,000 per annum
You will:
Co-ordinate the overall delivery of the Loneliness Prevention Project ensuring the project reaches and surpasses all targets, as set out in project plan.
Work with individuals and groups of people aged 65 years old to reduce loneliness and social isolation.
Work alongside statutory services to identify and receive referrals for the project.
Plan and deliver a range of mentoring activities including one to one and peer support to engage participants and support a positive change in behaviour.
Support participants to get involved in other local activities within the community.
You will have:
Two years’ experience of working in voluntary sector, health, or social care in direct contact with people, families and carers in a paid or voluntary capacity.
Experience of working with adults in a mentoring setting to achieve successful outcomes.
Proven track record of working with vulnerable adults and affecting positive change in behaviour.
Experience of working alongside key agencies across health and social care and the voluntary sector
Hold a safeguarding qualification or evidence of recent professional training.
